Request a Copy of Medical Records

To request a copy of your medical records from our office please complete and turn in a Medical Records Release (PDF) form. Your records will be ready to pick up within two business days.

Online Medical Records

We encourage our patients to access the Patient Portal  to view a limited portion of their medical records, and to send us NON-URGENT / routine messages.
The information available on this site currently includes:

• Active medication list
• Current and past medical problem list
• Recent laboratory results from labs that digitally interface with our records, after Doctor has reviewed results
  • (at this time only St. Charles Labs are viewable) (BMC or Interpath labs currently not available)
• Appointment reminders
